Help With Flu Report



Getting Started


Select the 'Sign Up' button from the Home page to create your user account.  You will be asked to supply information about yourself and to choose a username and password.  The username must be unique as well as your e-mail address.  This information is used to identify you as you enter Influenza Cases and to contact you in case you forget your password.  The user account information is not shared with any other entities or organizations.  Be sure to add facilities and providers for which you will enter Influenza Cases.  The providers and facilities you choose here will populate the drop down lists on the Add/Edit Influenza Case page.  If you cannot find a facility or provider, you can search the National Plan and Provider Enumeration System to find the National Provider Identifier.  You can also use that information to add a new facility or provider.  When you press the 'Save' button, your account information will be saved and you will be redirected to the Sign In page.  Use your new Username and Password to login.  You can optionally choose to remember your Username and Password as well as Automatically Sign In.


Influenza Cases


This is where you will Add Influenza Cases, Show Influenza Cases in Progress that you are currently working, and Show Completed Influenza Cases that you have submitted to the Arkansas Department of Health.  When you add a new flu case and select the 'Save for Later' button, you can then select Show Influenza Cases in Progress to edit the flu case and continue to work on it.  After you press the 'Save and Submit' button, you will no longer be able to edit the flu case.  You can continue to display the flu case by selecting Show Completed Influenza Cases.  The flu cases you enter belong to you.  No other user will be able to view or edit your flu cases.  If you do not want to identify the patient, use your medical record number instead of the patient's name.  The patient name and address are for you to identify your patients.  The Arkansas Department of Health is interested in the aggregate data, not individual patients.  City, county, and ZIP code information is useful in identifying concentrations of cases, though.


User Profile


Use the User Profile to maintain your user account information.  You may change your password and username here.  This is where you can add new facilities or providers to your drop down lists.  You can also change the dates when facilities/providers appear or disappear from your drop down lists and the order is which they appear.


Forgot Your Password


When you choose the 'Forgot Your Password' link, your user name and password will be e-mailed to you at the e-mail address contained in your user profile.  If your e-mail client supports TLS certificates, a web page will display.  If not, the email will contain an 'Unable to execute URL' message.  Just click the link or copy and paste the link into you browser to display the web page.
